That's a Hopkins & Holloway component, the company was bought by Fuji some years ago. Their fly guides were used on Sage rods for a long time, they had the first thin wire guides I was aware of. I know Angler's Workshop still carries their guides, the rest of the components and where Angler's gets the guides, I have no knowledge of.
I know about the component from seeing them on some British salmon rods.

I know bass pro used to have an eva foam style back in circuit from 2005-2007ish. You might be able to make one from something found at a Westlake Ace hardware, Nuts & Bolts, etc. I've never had luck finding items at Home Depot or Lowes. Maybe some type of foot made for furniture?

Fuji does not own H&H. Anglers Resource reps H&H in the US but there is no connection between Fuji and H&H.
