The tip ring size is specified by the OD of the ring.

So, in a nutshell, tip ring sizes are measured from the outside of the ring.

This is also the same thing that is used for their guide.

That is why, the guides that do not have inserts, have a larger hole, than the same guide or tip with an insert in the same.
